Admissions and registration
Have you planned an admission? Sign in half an hour beforehand with your eID or ISI+ card at a registration kiosk in the central reception (entrance 11).
Use the touchscreen on the registration kiosk to go through a few steps. This is quick and easy, but do not hesitate to ask a volunteer or staff member at the information point for help.
To complete your registration, the kiosks will direct you to the Admissions and registration desk. There, a staff member will go over all the information about your admission with you.
Admission documents
The staff member of Admissions and registration will give you the following documents:
- if you were not given one beforehand, an admission folder containing:
- the admission brochure for adults (1.33 MB) "pdf" or the admission brochure for children (2.74 MB) "pdf"
(currently only available in Dutch) - information about home medication
- a medication schedule (74.45 KB) "pdf"
- a checklist to prepare for your discharge (73.97 KB) "pdf"
- the admission brochure for adults (1.33 MB) "pdf" or the admission brochure for children (2.74 MB) "pdf"
- a copy of the admission declaration signed by you
Identification bracelet
After you register, the Admissions and registration staff will put an identification bracelet on you showing your name and date of birth.
- Check that your details on the bracelet are correct.
- Never remove your identification bracelet yourself. If you do lose it, you must ask for a new one.
You will then be directed to the nursing ward or the admissions lounge, where a nurse will give you further guidance.
Personal privacy code
To protect your privacy, you will be given a personal code that family and friends should use to ask the ward about your health condition. The code is on the admission form. The last four digits form your personal privacy code.
You decide to whom you give the code. Our staff only give information to family members and friends who can present your first name, surname and personal code.
