Have you planned an admission to hospital? Then consult this overview so that you do not forget anything on the day of admission.
For your registration
- Your eID or, if your child is admitted, the kids-ID or ISI+ card if you have one
- If applicable, your parking ticket, so that you can pay the reduced care rate at the ticket machine later on
- Adhesive notes from the health insurance fund
- If you have hospitalisation insurance, the necessary documents (insurance card, confirmation letter, etc.). Most insurance companies ask that you inform them in advance of the admission
- Your telephone number, mobile phone number, e-mail address
- Contact details of family, neighbours or other contacts
- Name, address and telephone number of your general practitioner
- Name of the doctor who refers you to UZ Gent
- If you use home care services: their name, address and telephone number
- For the maternity ward: marriage certificate or proof of recognition of the child
- In case of admission after a work-related accident or after an accident on the way to or from work: name and address of your employer, name and address of the insurance company, policy and file number
- When admitted for a clinical study: the reference letter you were given by the medical service
International patients should bring their proof of identity, insurance card from their insurance company and referral letter.
Medical data and medication
- Radiographs
- Results of previous examinations
- If applicable, a medication schedule (74.45 KB) "pdf". You can fill this in yourself or request it from your GP, home care nurse or pharmacist
- Medicines you are taking, in the original packaging
- (Copy of) certificates, e.g. certificates of established allergies
- Letter from your GP or doctor
- Results of the preoperative examinations
- If applicable, a completed pre-operative questionnaire
For your stay
- Nightgown, dressing gown, closed slippers or shoes
- Toiletries
- If applicable: anti-embolism stockings, insulin machine or glucose meter, crutches, support bandage or brace, etc.
