A completed medication schedule
Ask your GP or pharmacist to fill in a medication schedule for you. They can upload it on the CoZo healthcare platform or give it to you on paper. In the latter case, bring the schedule with you to your appointment. You can also scan it and upload it via My UZ Gent (click on Send file).
The completed preoperative questionnaire
The questionnaire contains important information for the anaesthetist. Be sure to fill it in. This can be done in several ways:
- Via My UZ Gent (click on Questionnaires).
- Via the email you receive.
- On paper (bring the completed questionnaire with you to the consultation).
Preferably complete the questionnaire digitally. Your answers will then automatically end up in your care file.
Medical reports of consultations and examinations
Recent examination results and reports from doctors who are not listed on CoZo: bring them with you on paper or send them to us digitally via My UZ Gent (click on the Send file button).
Permission for surgery and anaesthesia
A doctor can and may only anaesthetise and operate on you after you have given formal permission.
- In principle, you have already signed the form giving your surgeon permission to operate on you and handed it in at the outpatient clinic.
- The form giving permission to the anaesthetist to anaesthetise you is part of the preoperative questionnaire (both digital and paper versions).
