From children up to 15 years old
As a legal parent or guardian, you automatically have access to the portal for your children up to the age of 15.
You log in to My UZ Gent as usual. Next, you will be asked to choose the profile. Under 'For my child' select the name of your child and click on 'Confirm profile'.
From 16 years and older and other persons
For children over the age of 15 or other persons, you first need a mandate. This can be arranged in two steps. First, the mandator gives you the mandate, then you accept it.
Give mandate
- Go to the CSAM mandate system and click on 'Manage my healthcare mandates'
- Log in 'in your own name' to the online government
- Create a new mandate. For this you need the national registration number, name and address of the person to whom you are assigning the mandate
Take a mandate
- Go to the CSAM mandates system and click on 'Manage my healthcare mandates'
- Log on 'in your own name' to the online authority
- Accept the mandate you were granted
It may then take up to 24 hours for you to access the mandator portal. You log in to My UZ Gent as usual. Next, you will be asked to choose your profile. Select the name of the mandator under 'Under the mandate of' and click 'Confirm profile'.
